This role is essential in supporting residents' psychosocial needs, coordinating discharge planning, and ensuring compliance with state and federal regulations while promoting a resident-centered care environment. Essential Duties & Responsibilities Direct and oversee the Social Services Department, ensuring high-quality psychosocial care for residents and families Conduct psychosocial assessments, care plans, and ongoing evaluations in accordance with federal and state regulations Provide counseling and support to residents and families regarding adjustment, illness, discharge planning, and end-of-life care Coordinate and lead discharge planning efforts in collaboration with the interdisciplinary care team Act as a liaison between residents, families, facility staff, and community resources Assist with resident admissions, transfers, and discharges, ensuring appropriate documentation and follow-up Address resident and family concerns and grievances promptly and professionally Participate in care plan meetings, QAPI initiatives, and facility committees as required Maintain accurate, timely documentation in compliance with facility policies and regulatory standards Ensure compliance with CMS, state, and facility regulations related to social services Qualifications Bachelor's degree in Social Work or related field required Current Social Work license or certification as required by state regulations Minimum of one (1) year experience in a skilled nursing or long-term care setting preferred Strong knowledge of psychosocial needs of the geriatric population Excellent communication, organizational, and interpersonal skills Ability to work collaboratively with interdisciplinary teams Knowledge of Medicare/Medicaid regulations and discharge planning processes Physical & Work Requirements Ability to meet physical and sensory requirements of the position Ability to work in a fast-paced healthcare environment May require standing, walking, sitting, and lifting in accordance with facility needs Why Join Our Team?
