We are seeking a dedicated and empathetic Director of Social Services to lead and oversee programs that support the emotional, social, and behavioral well-being of our residents. Key Responsibilities Lead and oversee all social services programs under the direction of the Administrator. Conduct comprehensive resident assessments to identify psychosocial needs and available support resources. Coordinate community resources, referrals, and support services for residents and families. Collaborate with the Interdisciplinary Team (IDT) to align social services with individualized care plans. Assist residents and families with discharge planning, financial assistance, and community placement resources. Facilitate access to behavioral health services and psychosocial support programs. Participate in resident council and family meetings while advocating for resident rights and concerns. Educate residents and families regarding resident rights, advance directives, living wills, and related resources. Maintain compliance with all state and federal regulations and recommend policy updates as needed. Provide translation and interpretation assistance for residents with limited English proficiency when applicable. Maintain accurate, timely, and professional documentation in accordance with facility standards. Participate in surveys, audits, compliance initiatives, and quality improvement programs. Qualifications Bachelor's degree in Social Work or related human services field such as Sociology, Gerontology, Psychology, or a related discipline.

MUST HAVE

a minimum of two (2) years of experience as a Social Worker in a skilled nursing facility or healthcare setting. Strong leadership, communication, and independent decision-making skills. Compassionate and resourceful professional with the ability to work effectively with residents of varying cognitive levels.

MUST HAVE

experience with Microsoft Office, including Excel, Outlook, and PowerPoint, as well as PointClickCare (PCC). Ability to collaborate effectively with residents, families, healthcare professionals, and community agencies. Good standing with applicable state and/or national licensing or professional organizations. Commitment to continuous learning, innovation, and excellence in resident care.
