Job Description

Position Summary:
The Social Services Director is responsible for managing the psychosocial needs of residents in a skilled nursing facility. This role ensures that residents' emotional, mental, and social needs are met, and that discharge planning, resident rights, and advocacy are provided in compliance with federal, state, and facility policies.
Essential Duties and Responsibilities:
Conduct psychosocial assessments on all new admissions and readmissions within the required time frame. Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team. Serve as a liaison between residents, families, facility staff, and community agencies. Coordinate discharges, including referrals, transportation, home care, or alternative placement when needed. Address resident grievances and ensure the protection of residents' rights. Organize care conferences with residents and family members. Maintain documentation in compliance with state and federal regulations. Participate in facility Quality Assurance and Performance Improvement (QAPI) programs. Provide support and crisis intervention to residents and families as needed. Facilitate and participate in end-of-life planning and services coordination. Educate residents, families, and staff on psychosocial issues affecting care and quality of life. Maintain records, assessments, and documentation in EMR systems accurately and timely.
Qualifications:
Bachelor's degree in Social Work (BSW) required; Master's degree in Social Work (MSW) preferred. State licensure or certification (if required by state regulations). Minimum of 2 years of experience in a healthcare or long-term care setting preferred. Knowledge of federal and state regulations governing skilled nursing facilities. Strong communication, organization, and advocacy skills. Ability to handle sensitive and confidential matters with professionalism.
Working Conditions:
Primarily works in a skilled nursing facility environment. May involve extended periods of standing, walking, and interaction with residents and families. Occasional evening or weekend work may be required for emergencies or family meetings.
