What You'll Do:

Lead and support the Social Services department to ensure residents' emotional, social, and discharge-planning needs are met Partner with nursing, medical, administrative, and interdisciplinary teams Build strong relationships with residents, families, and community agencies Participate in assessments, care planning, and resident/family meetings Assist residents/families with navigating financial programs (Medicaid/Medicare) Help ensure compliance with all state and federal regulations

What We're Looking For:

Bachelor's degree in Social Work or a human services field At least 1 year of experience in a Skilled Nursing Facility (SNF) Compassion, strong communication skills, and a commitment to resident-centered care If you're ready to make a meaningful impact in a role that truly matters, we'd love to connect.
