• Responsibilities of the

Director Of Social Services:

• Plan and implement a comprehensive social services program, offering counseling and support services for residents and their families.

Evaluate and understand residents' medical-related social needs, formulating appropriate treatment plans within their total care plan. Coordinate and conduct family meetings, fostering ongoing discussions between residents, their families, and the treatment team. Maintain residents' rights while acting as a liaison between all involved parties. Coordinate discharge planning, assisting in the creation of organized discharge plans for all residents. Ensure residents and their families receive the highest quality service in a compassionate atmosphere that recognizes individual needs and rights.

• Requirements:

• Bachelor's Degree or higher, preferably in Social Work. Previous experience in Long-Term Care (LTC) or Skilled Nursing Facility (SNF) settings.
