This leadership role is ideal for someone who thrives on making a difference in the lives of seniors and their families, while also guiding a team of dedicated staff.
Key Responsibilities:
Oversee and coordinate social services for both short-term rehab and long-term care residents Conduct psychosocial assessments and care planning Serve as a liaison between residents, families, and interdisciplinary teams Ensure compliance with federal, state, and facility policies Support residents' emotional, mental, and social well-being through advocacy and guidance
Qualifications:
Bachelor's degree in Social Work or related field (LSW preferred) Previous experience in a skilled nursing or healthcare setting Strong knowledge of state and federal regulations (MDS, discharge planning, etc.) Excellent communication and organizational skills Leadership or supervisory experience preferred
