Overview Life Care Center of Burlington is seeking a highly skilled and dedicated Social Services Director (LTC) to lead our social work team within a long-term care environment. This leadership role requires a compassionate professional with extensive experience in behavioral health, clinical counseling, and case management. The ideal candidate will oversee all aspects of social services operations, ensuring comprehensive patient assessments, effective care planning, and compliance with healthcare regulations. This position offers an opportunity to make a meaningful impact on residents' quality of life through expert guidance and innovative social work practices. Duties Lead and supervise the social services team to ensure delivery of high-quality patient care aligned with facility goals and regulatory standards. Conduct comprehensive assessments of residents' psychosocial needs, including diagnostic evaluations and behavioral health screenings. Develop individualized care plans incorporating therapies such as cognitive behavioral therapy, applied behavior analysis, and motivational interviewing to support residents with diverse needs including Alzheimer's, PTSD, autism, addiction issues, and developmental disabilities. Coordinate with interdisciplinary teams to facilitate group therapy sessions, clinical counseling, and family counseling to promote emotional well-being. Manage documentation processes ensuring accurate medical records, HIPAA compliance, and timely updates within electronic medical record (EMR) systems. Oversee intake procedures for new residents and ensure seamless case management from admission through discharge or transition. Implement crisis intervention strategies to address urgent behavioral or mental health issues promptly and effectively. Skills Extensive knowledge of behavioral therapy techniques, including applied behavior analysis and cognitive behavioral therapy. Proven experience in child & family counseling, crisis management, and working with individuals with developmental disabilities or autism spectrum disorder. Strong background in home & community care settings as well as hospital environments. Expertise in managing mental health conditions such as PTSD and addiction through evidence-based approaches. Demonstrated leadership skills with supervising experience in social work or behavioral health teams. Proficiency in medical terminology, medical documentation practices, and EMR systems for data collection and reporting. Excellent communication skills for patient assessment, individual/group counseling sessions, and collaboration with healthcare professionals. Ability to handle complex cases involving diagnostic evaluation and psychotherapy while maintaining compliance with HIPAA regulations.
