This position is part of a strong nurse management team, utilizing a very high ratio of nurse managers to residents to oversee a small skilled nursing unit within a continuing care retirement community. The role is primarily weekdays, with a predictable on-call rotation, and flexibility to balance hours during the week.Benefits :

We are seeking a dedicated Admissions Coordinator to join our team! The Admissions Coordinator makes assessment for, plans and advocates for the residents of the Community so they can reach their highest practical level of physical, mental and psychosocial well-being. The Admissions Coordinator will be responsible for pre-admission assessment of referrals as well as discharge planning for all the residents of the Community. Has the ability to communicate effectively and advocate for resident centered/directed care to team members, families, physicians, allied health professionals and the at large community.Responsibilities:
Follows and upholds the Community's philosophy and goal to maintain, improve and/or enhance each resident's quality of care and quality of life Perform pre-assessment of residents as necessary. Attends and participates in facility meetings and in-service training programs as assigned. Participates in the nurse on-call rotation Promotes positive public relations with residents, family members, guests, and outside referral sources. Collects and assesses data relevant to resident's psychosocial needs, risk factors for psychosocial deterioration and responses to interventions. Completes assigned portions of the MDS. Maintains referral tracker. Coordinates admissions and discharge planning Reports monthly discharges to the Ombudsman. Coordinates and directs the care plan meetings. Facilitates the advanced directive decision-making process Coordinates grievance complaint process Coordinate and assist with resourcing and scheduling of allied health services, i.e. Dentists, Podiatrists, Medical specialists, etc.Qualifications:
Must be a graduate from an accredited school, be currently licensed RN or LPN in the state in which applying without encumbrance, and be responsible for maintaining professional licensure and CEU requirements. If the selected candidate is an LPN, continued employment is contingent upon enrollment and active progress in an accredited Registered Nursing (RN) program. The employee agrees to begin the RN program within six (6) months of the employment start date and to complete the program within its standard duration. Proof of enrollment and documentation confirming the program's expected completion timeframe must be provided upon request. Prefer 2 years of experience working in a geriatric setting.
