Director of Social Services

Director of Social Services We are seeking a highly motivated and compassionate Director of Social Services to join our team at Spring Creek Rehab in Murray, Kentucky. As a key member of our leadership team, the Director of Social Services will be responsible for overseeing the social services department and ensuring that our patients receive the highest level of care and support.

Director of Social Services Responsibilities:

Conduct assessments and screenings to identify patients' social, emotional, and psychological needs Develop and implement individualized care plans to address patients' needs and promote optimal health outcomes Collaborate with healthcare providers and other interdisciplinary team members to ensure that patients receive comprehensive and coordinated care Ensure compliance with relevant laws, regulations, and accreditation standards Participate in quality improvement initiatives and activities to improve patient outcomes and satisfaction

Director of Social Services Requirements:

Bachelor's degree in Social Work or related field 1 year of experience in healthcare Strong clinical and interpersonal skills Excellent communication, problem-solving, and organizational skills Ability to work effectively in a fast-paced environment and manage multiple priorities Strong knowledge of social services principles, theories, and best practices
