Wraparound Supervisor Cornerstone Youth and Family Services Baton Rouge, LA Job Details Full-time 1 day ago Benefits Paid holidays Health insurance Dental insurance Paid time off Cell phone reimbursement Vision insurance Qualifications First aid Full Job Description Position Summary The Wraparound Supervisor is a full-time exempt position providing leadership, coaching, and oversight to ensure high quality, individualized planning and effective service coordination for all enrolled youth and families. This role supports Wraparound Facilitators in delivering strengths based, family driven care and ensures that risk assessments, crisis plans, and Plans of Care are developed collaboratively and with fidelity to wraparound principles. This position reports to the Wraparound Program Director and provides direct supervision to Wraparound Facilitators, and collaborates with Independent Evaluators, Family Support Partners, Youth Support Partners, and Community Providers. DutiesResponsibilities Program OversightCare Coordination Ensures individualized planning and effective coordination of services for all youth enrolled in wraparound. Promotes family participation in identifying strengths, needs, and goals during the development of risk assessments, crisis plans, and Plans of Care. Serves as a liaison between the Statewide Management Organization, Wraparound Facilitators, and families to support smooth care planning and service coordination. Ensure each Plan of Care is completed, reviewed for quality and fidelity, and submitted to the Statewide Management Organization (SMO) prior to the expiration of the 30-day initial authorization period. Monitor authorization timelines to prevent service interruption. Ensure re-authorizations and updates are submitted timely in accordance with SMO requirements. Staff SupervisionCoaching Supervises up to 8 Wraparound Facilitators, providing ongoing guidance, consultation, and support. Observes Child and Family Team (CFT) and engagement meetings regularly and completes required observation documentation. Reviews all facilitator documentation including Plans of Care, contact notes, charts, and family narratives, offering timely feedback to improve quality and fidelity. Assures that plan of care development is completed, and plan is submitted to the Statewide Management Organization prior to the end of the 30-day initial authorization period. Administers competency assessments (brief and full) and uses results to develop collaborative coaching plans with each facilitator. Conducts group coaching sessions and partners with the Training Coordinator and Lead Coach to identify training needs and opportunities. Administrative Responsibilities Manages related tasks including approving time requests, assigning new referrals, reviewing documentation, and implementing corrective action plans when necessary. Verifies and processes flex fund requests and provides support to facilitators during high intensity family crises. Staff DevelopmentTraining Encourages professional growth and provides training opportunities on an on-going basis that reinforce Coordinated System of Care (CSoC) principles and values. Promotes a strengths based, collaborative team culture that supports high quality wraparound practice. CommunitySystems Collaboration Builds and maintains strong relationships with community partners to support multi system collaboration. Ensures effective interagency processes related to staffing, billing, communication, and service coordination. Quality AssuranceProgram Monitoring Monitors all aspects of wraparound service delivery including fiscal processes, claims management, payment workflows, and documentation quality. Supports quality improvement efforts by identifying trends, addressing gaps, and implementing corrective strategies. Qualifications Master's degree in human services, social work, behavioral science, or related field required. Minimum of 3 years of experience in human services, preferably across multiple systems. Relevant LDH approved alternative experience may substitute for degree; At least 2 years of supervisory or management experience with demonstrated success in community based programming. Must have expertise, knowledge and skills in the wraparound model and possess the ability to teach and develop those skills in the Wraparound Facilitator. Must have a high-degree of cultural awareness and the ability to engage families from different cultures, and backgrounds. A preferred supervisor/coach characteristic is an understanding of and experience with, different systems, including schools, behavioral health, child welfare, juvenile justice, health and others. Completion of the required training for WF supervisors/coaches. Meets all requirements for Wraparound Facilitator certification.
  • Provides 24/7 on call support to program staff; must be able to work flexible hours including evenings and weekends.
  • Meets agency auto insurance requirements and is able to drive extensively for business purposes. WAA Compliance Requirements Pass all required criminal background checks Be eligible for Medicaid/Medicare participation (no sanctions or exclusions)
  • Have no findings on the Louisiana State Adverse Action List
  • Complete TB testing and drug screening Complete AHA recognized First Aid, CPR, and seizure training (medical professionals exempt) Maintain all required Wraparound training and credentialing Benefits Overview Cornerstone Youth and Family Services offers a comprehensive benefits package for regular, full time employees.
