Social Services Director

Social Services Director Heirtage Manor West is seeking a compassionate and organized Social Services Director to join our nursing home leadership team. The Social Services Director is responsible for identifying and meeting the psychosocial, emotional, and social needs of residents while promoting resident rights, dignity, quality of life, and discharge planning.

Responsibilities include:

Providing social services support to residents and families Completing psychosocial assessments and care plan documentation Participating in care plan meetings and interdisciplinary team meetings Assisting with admissions, discharges, transfers, and room changes Supporting residents with adjustment to the facility Assisting with grievance follow-up and resident/family concerns Coordinating discharge planning and community resources Ensuring compliance with state and federal regulations Maintaining accurate documentation in the medical record Promoting resident rights and quality of life

Qualifications:
