The YMCA Youth & Family Services is looking for a self-motivated and dynamic professional to join our Y family. You will become part of a high functioning team that will help you learn skills throughout your career. You will make a difference everyday keeping children safe! As the Care Manager you will have bragging rights of being a part of a longstanding, reputable, and leading nonprofit organization that has a strong commitment to nurturing the potential of kids, promoting healthy living and fostering a sense of social responsibility. About the Job The Care Manager conducts youth-centered service meetings to determine client's strengths and needs and then what services are needed to address those needs. Provides general client services, group activies, programming, and case management in a Montgomery County Public School.
Position:
Care Manager Salary Range:
$50,000 - $52,000 Some of the exciting tasks you will do as a Care Manager include but are not limited to: Accepts referrals for social services and completes initial intakes Consults with county/community/agencies to identify and develop potential resources for client assistance Designs intervention strategy youth and/or youth's family to match appropriate community resource to the youth and family's needs and makes those connections Provides case management and monitoring services to youth/families Creates and executes student engagement activities for teens including weekly groups, workshops and activities Provides on-going social services (appropriate assistance during crises, may provide supportive counseling, food, clothing, housing, employment, transportation, financial assistance) Performs accurate recordkeeping including intakes, developing, reviewing and updating the Life works assessment, contact notes and electronic database entries
Qualifications and Skills:
Bachelor's degree in psychology, social work, sociology, criminal justice, family science or other human service-related field. 1-3 years' experience developing and administering teen centered programming Experience in working with diverse populations; and skilled at collaborating with multiple agencies Bi-lingual in English and Spanish preferred Relationship building (youth, parents, community, etc.) Clear, effective written and verbal communication Why You'll Love Working at the
