What You'll Do:
Lead and oversee the Social Services Department to support residents' emotional and psychosocial well-being Develop and implement individualized care and discharge plans Partner with residents, families, and the care team to ensure needs are met Provide guidance, support, and crisis intervention to residents and families Coordinate referrals to community resources and services Ensure compliance with all federal, state, and facility regulations Maintain accurate documentation, care plans, and social histories Supervise and support Social Services staff Participate in interdisciplinary team meetings and quality assurance initiative
What You'll Need:
Bachelor's Degree in Social Work or related Human Services field At least one (1) year of social services experience in a healthcare setting Strong communication and interpersonal skills Ability to lead, organize, and prioritize in a fast-paced environment Knowledge of regulatory requirements and resident rights Compassion, empathy, and a commitment to quality resident care Ready to Join Us?
