In this role, you will provide medically-related social services to attain or maintain the highest practicable physical, mental, and psycho-social well-being of each patient. Develop and implement case management services for all residents. Maintain contact with family (with resident's permission) to report on changes in health, current goals, discharge planning, and encouragement to participate in care planning. Maintain progress notes under federal and state guidelines for Long Term Care. Facilitate the building of relationships between residents and staff by teaching staff how to understand and support residents' individual needs.

Requirements:

BA from an approved Social Work school is required. Subacute and geriatric experience Minimum one year's experience with seniors. In a Supervisory capacity in a hospital, skilled nursing care facility, or other related medical facilities.
