Part-Time Bilingual (English/Spanish) - Social Service Specialist II/Case Manager -
AMAR Doula Children's Home Society Job Category Social Services Overview Description Overview:
The Case Manager/Doula back-up will work under the guidance of the AMAR Supervisor to provide carious services, including case management, prenatal education, intrapartum support, and postpartum care, to families associated with AMAR. Their objective is to support and empower families to maintain healthy practices and stable households. The Case Manager/back-up Doula will be part of a larger team responsible for planning and providing information and referrals to support program services. They will strive to create a safe and comfortable environment for the clients served within the program. The role will require a flexible schedule, local travel, and participation in on-call schedule. Candidates must be fully bilingual in both English and Spanish.
Responsibilities:
Offer case management services to clients. This includes help with Medicaid eligibility, education and career goals, legal matters, life skills, advocacy, and finding housing, social services, prenatal and pediatric care, and other community resources. Offer assistance to both mother and baby by providing physical and emotional support, educating on proper infant care, and promoting health attachment through responsive parenting, skin-to-skin contact, and breastfeeding. Offer additional assistance as a Doula before, during and after labor and delivery, even in the event of a perinatal loss. Help with recruiting and screening eligible clients for the AMAR program, which includes workforce development training, to ensure they can access the service they need. Create and send reports/letters to referral sources and community agencies when needed. Document all case management information clearly, accurately, and promptly. Establish and maintain relationships with different referral sources and community organizations. Get involved in promoting programs, marketing and engaging with the community. Create and present both verbal and written reports to staff and relevant professional organizations as necessary. Be on-call to respond call/texts and are available for support clients in between visits. Be on-call and available to leave for the hospital to attend and support the laboring.
Qualifications:
Education:
High School Diploma is required for this position.
Required Training and Certification:
Must be trained and certified in the Health Connect One (HC1) Community-Based Doula curriculum.
