Briarcliff Manor Center for Rehabilitation and Nursing Care is seeking an experienced, compassionate, and dedicated Social Services Director to join our leadership team. In this vital role, you will oversee all aspects of the facility's social services department, ensuring residents and their families receive the emotional, psychosocial, and practical support needed to achieve the highest quality of life and best possible outcomes. Key Responsibilities Direct and oversee all social services programs and services within the facility Coordinate discharge planning, financial assistance, and community resource referrals Ensure residents' psychosocial and behavioral health needs are addressed through individualized care planning Advocate for resident rights and provide education regarding advance directives and available services Participate in resident and family meetings to support communication and care planning Maintain knowledge of federal, state, and local regulations governing long-term care facilities Develop, implement, and update social services policies and procedures Collaborate with interdisciplinary teams to enhance resident outcomes and quality of life Support regulatory surveys, audits, and compliance initiatives Establish and maintain relationships with community agencies and healthcare resources Provide guidance and support to residents, families, and staff regarding social service concerns Maintain accurate and timely documentation in accordance with facility and regulatory requirements Qualifications Bachelor's degree required; Social Work or a related field preferred Minimum of two (2) years of social work experience in a healthcare, rehabilitation, or long-term care setting Prior supervisory or leadership experience preferred Strong knowledge of skilled nursing facility regulations and resident rights Excellent communication, leadership, organizational, and problem-solving skills Ability to build positive relationships with residents, families, staff, and community partners Compassionate, professional, and resident-centered approach to care Must meet all applicable health, background, and employment requirements Benefits & Perks Generous Paid Time Off (PTO) Medical and Dental Insurance Employee Recognition Programs Company Perks and Discounts Supportive and Collaborative Work Environment Professional Growth and Development Opportunities Apply Today Join a team dedicated to enhancing the lives of residents and their families through compassionate care and advocacy.
