Social Services Director - Long Term Care Gardens of Fairfax Cleveland, OH Job Details Full-time 1 day ago Benefits Disability insurance Health insurance Dental insurance 401(k) Tuition reimbursement Paid time off Vision insurance Qualifications Long-term care facility experience Full Job Description The Gardens of Fairfax is seeking a skilled and motivated individual to serve as our Social Services Director to join our dedicated team and help make a lasting impact in long-term care. The Social Worker is primarily responsible for planning, developing, implementing, and evaluating social service programs in accordance with federal and state standards, as well as established policy and procedures to provide medical and emotionally related social service needs for Residents of the facility.
Responsibilities include:
Conducting patient assessments to determine needs and develop individualized care plans Managing ancillary and psych services Manage and conduct care conferences as required Managing and completing PASRR and LOC's as required Assisting with discharge coordination and coverage in facility as needed Mentor Ridge is a fast-paced work environment. The successful candidate will have at least one year of experience in a long-term care environment and exhibit a compassionate and empathetic demeanor in caring for our residents.
