Complete psychosocial assessments and develop individualized care plans. Assist residents and families with admissions, discharge planning, and transitions of care. Advocate for residents' rights and emotional well-being. Coordinate community resources and support services. Participate in interdisciplinary care plan meetings. Ensure compliance with federal, state, and facility regulations. Provide support and counseling to residents and families as needed. Qualifications Bachelor's degree in Social Work or a related human services field required (Master's preferred). Long-term care or skilled nursing experience preferred. Knowledge of state and federal regulations related to long-term care. Excellent communication, organizational, and problem-solving skills. Compassionate, professional, and resident-centered approach.
