Job Description

Social Services Director For over 60 years, St. Anne Home, a ministry of the Felician Sisters of North America, has been committed to offering superior health care services to the senior members of our community. We are seeking to hire a Social Services Director . $65K to $75K annual compensation based on skills and experience.
Social Services Director Introduction:
Under the supervision of the Administrator, the Social Services Director is responsible for the planning, organizing, implementing, evaluating and directing of the Social Services program in accordance with current, federal, state regulations, industry best practices.
Social Services Director Qualifications:
Bachelor's Degree in Social Work, Human Services, Psychology, or Sociology.
Experience:
Minimum of 3 years of Social Work in Long-Term Care (nursing home) in PA.
Social Services Director Job Summary:
Guide facility in resident advocacy, protection and promotion of residents' rights. Manage department staff to allocate resources, supervise, plan and implement individual resident plans of care. Implement Social Work & Case Management initiatives and training of policies and procedures that govern social services. Prepare for the long-term care survey. Interact with State surveyors. Demonstrate assessment skills sufficient to evaluate residents' behavior, to collect data and to evaluate psychosocial needs, risk factors for psychosocial deterioration and residents' responses to interventions. Perform administrative requirements such as completing necessary forms and reports, including MDS and care plans. Maintain great relationships with community providers. Support Advanced Directives process and work closely with the medical and nursing team to initiate the desired resident/health care proxy's decisions.
