Job Description

Social Services Director Location:
Myerstown, PA We are seeking a dedicated Social Services Director to join our skilled nursing team in Myerstown, PA. The Social Services Director serves as a liaison and advocate for residents' social interests and is responsible for planning, organizing, implementing, evaluating, and directing the overall operations of the Social Services Department. Position Details Full-Time Job Functions Develop comprehensive social assessments and care plans identifying residents' medically related social and emotional needs with realistic goals and interventions. Actively participate in pre-admission and admission processes. Provide compassionate support to residents and families during adjustment, crisis, or special circumstances. Maintain accurate documentation of residents' behavioral and psychosocial status. Provide individual and group support services for residents and families. Establish and grow a volunteer program to meet social service needs. Orient residents and families to the facility and assist with transition and adjustment. Qualifications Bachelor's degree from an accredited school in Social Work or a related field required. Strong knowledge of social service practices, procedures, laws, and regulations in long-term care. Ability to work independently with strong interpersonal and communication skills.
