Strategy:
Full-Service Community Schools (FSCS) use a holistic approach to education, integrating academics with health and social services, youth development, and family engagement. These schools function as hubs of support by partnering with families, educators, and community organizations to remove barriers to learning and promote student success. The Team Manager is a key member of the FSCS team responsible for leading the implementation and oversight of targeted case management and support services for youth and families with complex needs. This role ensures that services effectively identify and address barriers by connecting families to school- and community-based resources. The Team Manager collaborates with healthcare providers, social service organizations, school districts, and families to improve health and wellness, academic achievement, and meaningful community engagement. This role also ensures service quality, strengthens partnerships, and maintains fidelity to service models. The Team Manager supervises Resource Specialists responsible for delivering direct case management services to high-need youth and families across partner school districts, health clinics, and community-based organizations. Key Responsibilities Service Oversight & Implementation Lead implementation and management of case management and support services across all school district and community partnerships. Provide trauma-informed supervision and coaching that supports staff growth, well-being, and alignment with culturally responsive, evidence-based practices. Ensure services are high-quality, data-driven, and responsive to identified family needs. Oversee service data, ensuring accurate, timely documentation, and reporting in the PACS data management system. Manage site-level budgets and procurement processes in compliance with organizational policies. Build and maintain partnerships with community providers and support the PACS Provider Council. Team Management & Development Foster a culture of accountability, collaboration, and continuous improvement. Monitor performance metrics; provide coaching, feedback, and professional development support. Facilitate team meetings that promote communication, learning, and alignment. Partner with leadership to recruit, hire, onboard, and retain strong staff. Leadership & Strategic Growth Identify and advance opportunities to expand partnerships and enhance service delivery. Contribute to PACS organizational strategy and leadership initiatives. Support the Family Advisory Council and elevate family voice in program design and implementation. Working Environment & Schedule Travel up to 40% for local and occasional national meetings. Ability to occasionally lift up to 30 pounds. Valid PA driver's license and reliable transportation required. Flexible schedule, including some evenings for meetings and community events. Hybrid work environment across PACS offices, schools, partners, and community settings. Qualifications Required Bachelor's degree in Social Work, Human Services, Public Health, Education, or related field. 3-5 years of experience in case management, social services, education, or community-based programming. At least 1-2 years of supervisory or team leadership experience. Demonstrated experience working with youth and families facing complex needs. Strong knowledge of community resources and systems navigation. Experience using data systems for service tracking and reporting. Strong interpersonal, communication, and relationship-building skills. Ability to collaborate effectively across schools, families, and community partners. Commitment to equity, cultural responsiveness, and trauma-informed practice. Preferred . Experience in Full-Service Community Schools or similar integrated models. Knowledge of Pennsylvania education and human service systems. Experience with using Microsoft 365 products
