Licensed Social Services Director 2.9 2.9 out of 5 stars 1330 India Hook Road, Rock Hill, SC 29732 Full-time Westminster Towers 23 reviews Full-time Westminster Towers is a not-for-profit Licensed Continuing Care Retirement Community (CCRC) providing senior housing to Independent Residents, Assisted Living Residents, and Skilled nursing residents. We also provide in home services and home health services. We currently have a great opportunity for a Licensed Social Worker to be a support to our Independent, Assisted Living, and Long Term Care Residents. You will support the emotional, cognitive, physical, interpersonal, and spiritual needs of the residents. The role includes facilitating the transition of care within the continuum of care on campus. Maintaining and enhancing optimal living while promoting acceptance and adaptations to those experiencing decreased capacities. This is a position for a mature individual with a wide skillset who will provide a variety of services, including a combination of case management, clinical practice, group work, and project initiatives to best meet the needs of individual members and the community as a whole. The ideal candidate will possess a passion for working with an intergenerational population and building support systems for residents. Quality customer service is a top priority for everyone at Westminster Towers.
RESPONSIBILITIES
Works with all available resources to arrange for services to meet residents' needs and facilitate their adjustment Makes routine contacts with staff and/or residents Participates in resident admission and discharge and provides necessary documentation, as needed. Coordinates transfer of a resident from one level of care to another. Actively participates in transitional care meetings Occasionally will document and chart care plans, assessments, results, meetings, and changes Meets and counsels with residents and families as needed. Provides in-service training to staff and residents Performs necessary administrative functions, record-keeping, correspondence, and communications Develops and maintains community resources and contacts Keeps educational credentials and professional certificates current Develops goals and objectives annually Develops and maintains outside professional contacts
REQUIREMENTS
LSW required Minimum of 5 years of experience working with a nonprofit CCRC Strong collaborative, analytical, and problem-solving skills are required. Masters in Social Work is preferred
