Resident Advocate Manager & Discharge Planner Cedar Health & Rehabilitation | Cedar City, Utah Be the Voice, Guide, and Champion for Our Residents At Cedar Health & Rehabilitation, we believe every resident deserves more than great clinical care. They deserve someone who advocates for their goals, supports their families, and helps them navigate life's transitions with dignity and compassion. We're seeking a Patient Advocate Manager to lead our Social Services department and play a key role in enhancing the lives of our residents. This is an opportunity for a compassionate leader who enjoys building relationships, solving problems, and making a meaningful difference every day. Why This Role Matters As the Patient Advocate Manager, you'll serve as a trusted resource for residents, families, and staff. You'll help residents navigate admissions, care planning, insurance and financial resources, discharge planning, and the emotional challenges that can come with healthcare transitions. You'll lead a department that directly impacts resident satisfaction, quality of life, and overall wellbeing. What You'll Do Lead and oversee the Social Services department. Advocate for residents and families throughout their stay. Assist in care planning, and discharge planning activities. Assist residents and families with Medicare, Medicaid, and other community resources. Develop meaningful relationships with residents, families, healthcare providers, and community partners. Collaborate with nursing, therapy, dietary, and leadership teams to ensure exceptional resident experiences. Maintain compliance with state and federal regulations related to long term care. Participate in quality improvement initiatives and resident satisfaction efforts. Coach, mentor, and support department team members.
What We're Looking For Required:
High School Diploma or equivalent. Strong communication and relationship building skills. Ability to manage multiple priorities in a fast paced environment. Compassion, professionalism, and a resident centered mindset.
Preferred:
Associate's or Bachelor's degree in Social Work, Human Services, Healthcare Administration, or a related field. Experience in healthcare, skilled nursing, social services, case management, admissions, customer service, or resident advocacy. Leadership or supervisory experience. Who Thrives Here?
