Job Description
First Choice In-Home Care is seeking a full-time Social Services Case Manager to manage a diverse Client caseload consisting of vulnerable and/or at-risk minors, adults and seniors with a primary focus on consolidating the service recipients' care team through the addition of qualified care providers. Our aim is to enable our clients to continue living in the comfort and safety of their home setting, versus becoming homeless or transitioning to an institutional living environment, through establishing and then sustaining their ongoing, in-home care team. Our ideal candidate is able to prioritize our Client's health and safety, is team oriented, responsible, reliable and accountable, has a passion for helping others that shows in many ways and feels a strong motivation to undertake challenging work for the benefit of vulnerable and/or disabled children, adults and seniors. First Choice is licensed by the Washington State Department of Health (DOH) and contracted with the Washington State Department of Social and Health Services (DSHS) to provide personal care services to our population of developmentally disabled children & adults, individuals with mental health disorders, veterans and seniors. We provide services to both State funded (Medicaid) and Private clients living in King, Pierce and Snohomish, Clark, Cowlitz, Kitsap and Spokane counties. We are an established and still growing Agency managing the care of over 3,100 vulnerable community members and are now hiring qualified, passionate and motivated staff members into our King County Case Management Department.

The position is full-time, Monday-Friday 8:30am-5pmBenefits:

Retirement Plan Annual Pay Increases Qualifications, Education, and Experience:
Bachelor degree with emphasis: Psychology; Social Services, Social Work, Sociology or related field is preferred but not required. Experienced DSP's are encouraged to apply. Previous professional or volunteer Social Services experience preferred but not required. Previous management and/or leadership experience is a plus. Ability to work effectively both independently and in a team environment. Excellent communication skills, enjoys creating new relationships with Clients, assigned staff, partners and co-works. Ability to work effectively both as a member of team and independently. Experience working or volunteering with disabled and/or vulnerable children, adults, and seniors is a plus. Ability to problem solve and to pro-actively seek solutions. Attention to detail and strong organizational skills. Responsible driving record, valid automobile insurance and ability to pass a criminal background. Extensive training for all essential functions is provided. Key Responsibilities & Competencies; include but are not limited to: Manage a client caseload comprised of 50-90 minors & adults with developmental disabilities as well elderly and aging individuals. Establish Client Care Teams, coordinate regular care regiment, assign and direct care providers. Attend bi-weekly department meetings. Participate in weekly intermediate team meetings. Ensure Client health and safety is maintained through comprehensive supervision and oversight. Coordinate with Human Resources Department, Training Department and Fiscal Department. Conduct one-on-one Intake Assessments with new Clients. Conduct semi-annual home visit meetings with existing Clients Complete administrative reports as needed and within stated deadline. Utilize proactive and creative problem solving methods to address the items arising within assigned caseload. Ability to prioritize team work and contribute to pressing needs outside of dedicated caseload assignment.
