Skilled Nursing facility is seeking an experienced Director of Social Services professional to join a high-volume rehabilitation setting. The ideal candidate will bring strong SNF experience, including managing the social services discharge process within a fast-paced rehab unit, along with the appropriate qualifications required for the role.

Qualifications:

Bachelor's Degree in Social Work or a Bachelor's Degree in Human Services field, including but not limited to Sociology, Special Education, Rehabilitation Counseling and Psychology.
