What You'll Do:
Lead and oversee the facility's social services program Partner closely with residents, families, interdisciplinary team members, and community resources Coordinate discharge planning and transitions of care Advocate for resident rights, needs, and overall well-being Help drive positive resident outcomes and an exceptional resident experience
What We're Looking For:
Bachelor's degree in Social Work or a related field (required) Strong communication and relationship-building skills Professionalism, initiative, and attention to detail Ability to manage multiple priorities and follow through on commitments A team player who is passionate about serving seniors and making a meaningful impact This is more than a job it's an opportunity to help shape the future of resident care at a facility that is moving forward.
