  • Full-time Social Services Coordinator The primary purpose of your job position is to assist in planning, organizing, implementing, evaluating, and directing the overall operation of the Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Social Services Director and/or Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.
Responsibilities Participates in discharge planning, development and implementation of social care plans and resident assessments Interviews residents/families to obtain social history Reviews departmental complaints and grievances from residents and personnel and prepare written reports to Administrator of action(s) taken Ensures that all charted progress notes are informative and descriptive of the services provided and of the resident's response to the service Coordinate resident/family in planning of objectives and goals for the resident Administrative duties to include but not limited to submission of necessary forms and reports Distribute resident funds to the resident's daily. Reconcile with BOM at the end of each day. Qualifications High School diploma or GED. Associate's or bachelor's preferred. Excellent communication skills Ability to work independently Must possess the ability to deal tactfully with personnel, residents, family members
