The ideal candidate will have prior hospice experience, strong relationship-building skills, and the ability to educate referral sources and families about hospice services with professionalism and empathy. Position Summary The Community Liaison is responsible for developing and maintaining professional relationships with hospitals, physicians, skilled nursing facilities, assisted living facilities, case managers, and community partners to support appropriate hospice referrals and admissions. Responsibilities Develop and maintain referral relationships within the community Educate healthcare professionals, patients, and families regarding hospice services and eligibility Conduct facility visits and community outreach Collaborate closely with clinical and admissions teams Follow up on referrals and assist with coordination of care Maintain knowledge of hospice regulations and services Participate in marketing and community events as needed Qualifications Hospice experience required Previous experience in hospice marketing, admissions, or community liaison role preferred Strong communication and interpersonal skills Knowledge of hospice eligibility and Medicare guidelines preferred Ability to build professional relationships and maintain referral sources Professional appearance and dependable transportation required Valid driver's license and auto insurance required What We Offer Competitive pay Supportive team environment Opportunity to make a meaningful impact in the community Flexible schedule Growth opportunities
