The Housing Navigator plays a crucial role in assisting individuals and families in securing stable housing. The Housing Navigator will play a crucial role in providing comprehensive linkage plans, advocacy, and community-based resource referrals for clients experiencing housing instability. This position involves working collaboratively with community partners and faith-based programs to ensure the long-term wellness and recovery of clients. The ideal candidate will possess strong crisis management skills and a comprehensive understanding of housing regulations, including Fair Housing laws and Section 8 programs.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Client Support (70%)
  • Provide navigation services, implement linkage and housing plans, and administer the Community Screening Tool.
  • Coordinate with community resources to promote physical and mental well-being, and self-sufficiency.
  • Conduct site visits and meetings with housing agencies and property owners.
  • Conduct land-lord engagement activities and serve as a liaison/consultant to property owners.
  • Assist individuals with case management services, navigating housing resources, and securing housing with an appropriate level of support.
  • Participate in all case-conferencing related to client work and progress.
  • Collaborate with the internal team to develop treatment plans for clients to facilitate placement in appropriate programs or housing settings.
  • Maintain up-to-date knowledge of available community resources and conduct land-lord engagement activities.
  • Transport clients to appropriate services as needed.
  • Link clients to appropriate public transportation services. Documentation and Data Management (10%)
  • Gather required client documentation and perform timely data entry.
  • Enter data into the Homeless Management Information System (HMIS), Community Screening Tool, and CHIP's tracking systems.
  • Evaluate and adjust case plan goals as needed.
  • Conduct and interpret client assessments and outcomes. Program Development/Community Relations (10%)
  • Attend independent Living home visits and other relevant meetings.
  • Collaborate with community partners to enhance resource availability.
  • Assist with program development and strategic planning.
  • Prepares presentations: Participates in meetings, discussions, committees, taskforces, develop goals, tasks, and measurement outcomes for housing related programs and services.
  • Provide responsive, high quality service to city employees, representatives or outside agencies and members of the public by providing accurate, complete, and up-to-date information, in a courteous, efficient, and timely manner.
  • Maintain good line of communication with Housing Locators, the Coordinated Entry Systems, and other housing support staff. Program Marketing (10%)
  • Assist with design and coordinate the production and delivery of program marketing and collateral materials including but not limited to event flyers, online registrations and social media.
  • Foster relationships with community partners, service providers, and other stakeholders to enhance collaboration and resource sharing.
  • Participate in relevant community meetings, coalitions, and forums to stay informed about emerging issues and opportunities for collaboration.
  • Provide support at and attend CHIP community events.
  • Take pictures at events to support the CHIP marketing.
  • Other duties as assigned.

QUALIFICATIONS
Incumbents must demonstrate the ability to follow policies and procedures as established in the Community Health Improvement Partners Employee Handbook. Incumbents will also contribute to a positive work environment by behaving and communicating in a manner such that they get along with all constituent groups, co-workers, and management. In addition, the education, experience, and other skills listed below are representative of the knowledge, skills, and/or abilities required.
EDUCATION
  • GED or High School diploma required.
  • 60 units of coursework in a related field or AA degree in social work or human services-related field preferred.
WORK EXPERIENCE
  • Placement techniques related to homeless adults
  • Knowledge of social work principles and practices.
  • Knowledge of rules and regulations governing the provision of housing, health, and social services to people who are homeless.
  • Knowledge of the City and County of Fresno's policies and procedures governing the provision of housing, health, and social services to people who are homeless.
  • Knowledge of the current trends and issues in the field of homelessness.
  • Knowledge of employment planning, training, and job readiness techniques.
  • Familiarity with public social service programs and regulations.
  • Understanding of HUD regulations and city/county intake processes.
  • Telephone, office, and online etiquette
EQUIPMENT AND APPLICATIONS
  • Proficiency in MS Office, including Word, Excel, and Outlook as well as Zoom
  • Working knowledge of all standard office equipment
  • Ability to work with diverse populations and engage clients.
  • Excellent communication skills, bilingualism is a plus.
  • Strong organizational and time management skills.
  • Ability to work independently and as part of a team.
