Job Description
This role oversees the Housing Support team ensuring that program goals are met and align with the organization's mission. This position is responsible for training program staff, quality assurance, and housing plan oversight and building relationships with stakeholders. The Housing Navigator/Lead Care Manager will play a key role in driving program success and supporting organizational growth.
Enhanced Care Management Program:
Responsible for interfacing with Clinical and ECM teams where appropriate, and operationalizing and effectively delivering Housing Services as defined by the managed care plan contracts, as well as DHCS requirements. Oversees Housing Care plans, participates in IDT meetings, and ensures appropriate and timely progress occurs.Housing Services:
Responsible for operationalizing and effectively delivering housing navigation, housing sustaining and housing deposit services as required by the managed care plan contracts as well as DHCS requirements. Effective management of CES and HMIS as well as an in-depth understanding of HUD, Voucher, Bridge Housing, Emergency Shelter, and other resources and systems. Participation with local CoC and stakeholder sessions required. Effective management of partnerships and referral sources such as United Way 211, through the FCES program, ensuring timely access to critical services are provided to families in need.Other Programs:
Effectively implement Mentorship Programs, Life Skills curriculum, Educational Opportunities, CHW Training, and Employment resources as defined by our mission to further support sustainable housing. Key Responsibilities Program Development and Management Support the design, planning, and implementation of programs to achieve organizational and program-specific goals. Oversee daily program operations, ensuring efficiency, effectiveness, and alignment with strategic priorities. Continuously evaluate and improve programs based on data and feedback, addressing gaps and identifying areas for growth. Develop and update standard operating procedure tools and job aides to standardize program practices and ensure compliance with best practices. Team Supervision and Development Support the team with training, supervise and support program staff, fostering a collaborative, mission-focused environment. Ensure all staff meet requirements for background, and complete successful onboarding, and training as required by State of California. Conduct regular performance evaluations, provide coaching, and support staff professional development to enhance team capabilities. Foster teamwork, communication, and accountability across program teams to achieve high standards of service and results. Program Evaluation and Reporting Collect and analyze data on program activities, participant outcomes, and performance metrics to evaluate program effectiveness. Prepare reports for funders, the board of directors, and other stakeholders, highlighting program accomplishments, challenges, and impact. Use data insights to inform strategic decisions and refine program goals, ensuring continuous improvement.- Support the implementation of technology and tools to ensure compliance with managed care plan requirements, as well as regulatory requirements of all programs.
Education:
Bachelor's degree in related field, examples are; Psychology, Nonprofit Management, Public Health/ Administration, Business Administration. Experience in lieu of education will be considered. Preference is given to persons with Housing Experience and/or Lived Experience.Experience:
Minimum of 3 years of experience in a leadership role providing Housing First approaches, demonstrating successful engagement and housing, building housing support teams, developing tools, and supporting programs.Case Management:
Demonstrated experience of 2+ years in Housing Support services.Financial Acumen:
Strong financial management skills, with experience in budgeting, reporting, and compliance.Leadership & Management:
Proven ability to lead, inspire, and manage a team with a collaborative and supportive approach.Communication:
Excellent interpersonal, public speaking, and written communication skills. Bilingual (English/Spanish) preferredCommunity Engagement:
Strong networking skills and the ability to build lasting relationships with community members and partners.Compliance Knowledge:
Familiarity with ECM, Community Supports, Medi-Cal, Medicare compliance requirements is highly desirable. Key Competencies Strategic thinking and problem-solvingCES, HMIS
