POSITION SCOPE
This position will provide community support and individual interventions based on assessment and evaluation; may assist in providing linkage and referral based on individual need and as required by the Community Support Program/Recovery Pathways. Responsibilities will primarily consist of the provision of direct care support; generally consist of guiding, assisting and supporting individuals who have complex psychiatric, substance abuse, medical, social and legal issues to improve daily living skills, improve level of functioning, enhance independence and promote recovery.
ESSENTIAL RESPONSIBILITIES
May assist in the provision of community and social support services for individuals. May assist in providing input into rehabilitation assessment, reassessments and in the review/update of individual recovery plans. Engages consumers and works in partnership to increase or maintain involvement in learning, working and social activities. With assist, provide best practice model interventions for consumers in continuing education, employment, and socialization. Develops relationships with clinical, educational, vocational, and other social service agencies in order to effectively support consumers in the community with increasing levels of independence. May complete documentation, or other required paperwork based on funder/program/agency requirements. Provides transportation to consumers as appropriate utilizing personal vehicle for agency business. Attends and participates in all required meetings. Adheres to all Chrysalis Center, Inc.'s policies, procedures and maintains professional standards at all times. Performs other duties as assigned.
KEY COMPETENCIES
Maintains operational knowledge of human services principles, assists in determining appropriate interventions for clients. Understands and abides by all agency policies and procedures. Completes and maintains documentation and required paperwork for all assigned consumers on time. Manages all relationships on behalf of agency and consumers in a professional and respectful manner. Able to solve practical problems and deal with a variety of variables in situations with limited standardization. Proficient with use of computer i.e. Microsoft Office, Electronic Health Record, etc., filing skills and other administrative skills. Skilled at individual and group-based interventions. Demonstrates excellent organizational, time management skills. Effectively communicates and presents information clearly and in a concise manner in verbal and written format. Demonstrates excellent interpersonal skills including professional demeanor and positive, service orientated attitude toward all internal and external customers. Demonstrates ability to adhere to strict Confidentiality and HIPAA Guidelines. Provides guidance, assistance and support to consumers as well as works collaboratively with Chrysalis Team.
MINIMUM QUALIFICATIONS
Peer Support Certification is required, plus one to two years of experience in mental health and substance abuse fields. Associates or bachelor's degree preferred. Recovery University certification a plus and may be considered in lieu of one-year experience. Experience providing residential and community services. Proficiency managing Microsoft and other computer applications used for tracking data and reporting outcomes. Easily adapts to change and competing priorities. Ability to de-escalate crisis with professionalism and sound judgment. Possess and maintains a valid Driver's License, reliable vehicle, current registration, and insurance. Ability to effectively interact with consumers and members of various teams within and outside the agency. Peer Support Specialist Certification Required Bi-lingual Spanish, preferred.
