Transitional Housing Specialist

Clearwater, FL We are offering the Transitional Housing Specialist a $500 sign-on bonus for new employees. WHAT YOU WILL DO
The Transitional Housing Specialist is a non-exempt position responsible for assisting individuals with Severe and Persistent Mental Illnesses (SPMI) and/or Co-occurring disabilities in transitioning into independent living in the community. The Transitional Housing Specialist may provide case management, linkage to community resources, and ongoing support to ensure clients can maintain the least restrictive level of care. Maintain an active caseload consisting of community-based clients and link these clients with the necessary support services required for maintaining independence
Coordinate the service planning process with the primary goal of assisting the client with achieving independent living
Prepare assessments and any related social, financial, or medical history reports or forms as they pertain to the client's needs
Utilize de-escalation skills to ensure client safety, including the development of a safety plan
Schedule and transport clients to community appointments
Establish a relationship with the client, family/guardian, significant other, and all other service providers connected to the client
Perform all other duties as assigned

WHAT YOU WILL NEED
Bachelor's degree in human services or related field
Previous experience working with Severe and Persistent Mental Illnesses (SPMI) and/or Co-occurring disabilities, preferred.
Must have computer experience, which includes Microsoft Office Suite and Electronic Health Records database
Must possess strong oral and written communication skills
Must obtain/maintain CPR and First-Aid Training within 90 days of employment
Must have a valid driver's license with current auto insurance coverage
