Since 1978, Gateway has been committed to providing compassionate services to those lost in the despair of addiction. Adolescent - Community Engagement Coordinator The Adolescent Community Engagement Coordinator (ACEC) provides community engagement and care coordination services for our adolescent patients who are in an episode of service, including their guardians and community partners. This individual works closely with the community providers, schools, Gateway peer, medical, and clinical staff while ensuring patients are provided appropriate services to assist with recovery. The ACEC may provide person and family-centered assessments, and care management plans and assist patients in connecting to the needed services and resources to assist them in achieving their own personal recovery goals.
ESSENTIAL FUNCTIONS
Thoroughly understands adolescent services and those offered at Gateway, including other providers in the community. The community engagement coordinator will work with community partners, local businesses, social clubs and groups, local government, and groups with school-age children to facilitate 30-minute to 1-hour presentations. The community engagement coordinator is responsible for providing supportive services to adolescents and their families. Ensuring linkage to supportive services in the recovery network. Assisting with linkage to care to treatment services, adolescent treatment engagement, and care coordination, while working to mitigate possible future arrest or criminal charges for the adolescents in our area. Involve the patient's peers and natural supports (family, friends, sponsor, and other community members) as requested and appropriate. Facilitate communication among the patient's providers and natural supports to assist them in achieving their recovery goals. Advocate for patients to receive services and resources necessary to assist them, including services needed when they are discharged from services. Support patients in transition from treatment services to the community. Providing warm hand-offs to service providers. Adhere to guidance on care coordination best practices, including understanding the core competencies for care coordination. Maintains current, accurate information in each record to include all activities, contacts, and communications. Obtains consents and releases of information when indicated. Document all services provided in a complete, timely, and accurate manner. Act as a member of a multidisciplinary and collaborative team with other healthcare professionals to assist in providing the best care possible for the patient. Establish a consistent and open relationship with all Gateway programs and staff. Provide excellent customer service to patients, their natural supports, other service providers, Gateway team members, community members, the managing entity, and any other person encountered in a professional context. 1Provide regular updates to the Director of Adolescent Services regarding community engagement, presentations, and outcomes. Participate in scheduled meetings, including clinical staffing, staff meetings, and collaborative meetings with other providers. Understand the various resources available in our community to assist with patient needs Ensure quality internal and external customer care and service. Demonstrate flexibility as a team member and promote a positive working environment. Provide all services according to insurance, DCF licensing, managing entity, SAMHSA, and CARF accreditation guidelines and document all services within the required timeframes according to organizational policies and procedures and maintain patient confidentiality according to
HIPAA, 42 CFR
Part 2, and other state and federal laws. Maintain ethical conduct regarding dual relationships, professional standards, and organizational values. Provide services with cultural and linguistic competency. Participate in program staff meetings, medical staffing, and other inter-organizational collaborations Other duties as assigned.
EMPLOYMENT STANDARDS
Must be eligible through DCF and AHCA with Level II Background checks throughout employment. Must abide by the organization's Code of Conduct, policy and procedures, and state certification code of ethics. The ability to comply with equal employment opportunity requirements. Must complete all in-person and Relias training assigned during probation and throughout employment. Must practice universal precautions as part of regular job duties whenever applicable. Timely and regularly complete all required documentation, reports, and other organization-related paperwork. Must be able to perform assigned work independently with minimal supervision and make decisions using appropriate judgment. Maintain the work area clean and orderly, ensuring safe operating conditions. Must be able to take direction and guidance from managers and supervisors respectfully and function as an effective team member. Must have the ability to comply with a workplace of dignity and respect.
QUALIFICATIONS EDUCATION/EXPERIENCE CERTIFICATION/LICENSURE
  • Bachelor's Degree in Counseling, Criminal Justice and other related Human Services areas. Experience in mental health and/or substance use disorder programs is preferred.
  • Experience in substance use and/or mental health disorder programs is preferred.
  • Valid driver's license with a safe driving record to meet standards for the organization's insurance requirements. SKILLS
  • Strong time management and organization skills.
  • Excellent communication skills.
  • Ability to recognize and respond to emergencies and life-threatening situations appropriately.
  • Ability to work in a fast-paced environment.
  • Ability to relate to adolescents, adults, families, referral sources, and internal Gateway team members and possess requisite competencies to effectively understand and provide appropriate services to the patient population served.
KNOWLEDGE
  • Knowledge of the recovery process and 12-Step programs preferred.
  • Knowledge of adolescent and adult behavior regarding family systems preferred.
  • Knowledge and understanding of cultural competencies to establish and maintain strong working relationships with culturally diverse patients, their families, caregivers, and a wide range of community agencies and organizations.
  • Moderate computer skills, such as inputting 35 wpm, using Microsoft Office Suites, and an Electronic Health Record.
