• The Social Services Specialist responsibilities include:
• + Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community + The Social Service Specialist II provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning + They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs + Possess a clear understanding of discharge and transition planning and linkage with community resources + Demonstrates skills in planning, organizing, and managing multiple functions and complex processes •Position details:•+ •

Certifications and Licensures:

• + Preferred ACM (Case Management) or + Preferred CCM (Case Manager)

• Education:

• + Required Masters in Social Work, Sociology, or Related Field + Strongly Preferred Masters Degree in Social Work

• Experience:

• + Required 6 months Internship or + Required 1 year Social Work in an acute care or hospital setting
