  • Full-time Overview The Community Liaison serves as the primary relationship builder and ambassador for the Home Health and Hospice agency.
This position is responsible for developing and maintaining strong referral partnerships with physicians, hospitals, skilled nursing facilities, assisted living communities, case managers, social workers, and other healthcare professionals. The Community Liaison educates the community about available Home Health and Hospice services, promotes agency growth, and helps ensure patients receive the appropriate level of care at the right time. Responsibilities Develop and maintain referral relationships with healthcare providers and community partners. Conduct routine visits to physician offices, hospitals, nursing facilities, assisted living facilities, and other referral sources. Educate healthcare professionals, patients, families, and community organizations about Home Health and Hospice services. Identify opportunities for patient referrals and facilitate smooth transitions of care. Collaborate with clinical leadership to ensure referral sources receive timely communication and support. Represent the agency at community events, health fairs, networking meetings, and professional organizations. Maintain accurate records of marketing activities, referral contacts, and business development efforts. Monitor referral trends and provide regular reports to agency leadership. Ensure all marketing and outreach activities comply with Medicare, Medicaid, HIPAA, and agency policies. Requirements Experience in healthcare marketing, business development, sales, community outreach, or healthcare operations preferred. Knowledge of Home Health and/or Hospice services preferred. Excellent communication, presentation, and relationship-building skills. Ability to work independently and manage multiple priorities. Strong organizational and documentation skills. Valid driver's license, reliable transportation, and proof of automobile insurance. Proficiency with Microsoft Office and electronic documentation systems.

$60,000.00
