Job Description

Previous Applicants Need Not Apply Two Years Fixed Term Here in Sunderland, we are diversifying how we help and support people who become homeless. We have realigned the Housing Options Team to ensure the team is well placed to meet the needs of all people who are street homeless and those threatened with homelessness. We will work to ensure that all move to and sustain independent living. For many this journey is not a straight line, therefore we have increased the team and have added more diverse roles to ensure people are supported at every stage of their journey to a safe and secure home. I'm sure you will be inspired by our passion and see that we have shaped a great team structure which will help homeless clients to move on with their lives, as well as being equipped to sustain a safe home. The Health Navigator will work with homeless clients with health needs at various stages of their journey to independent living. You will be based within the Council's Housing Options Team and collate a caseload of clients whose health needs need to be assessed and supported to allow a move on to independent living. You should have a knowledge and understanding of the challenge's vulnerable homeless clients face and the ability to identify any health or safeguarding needs. You should also have a good track record of creating pathways and building partnerships with both internal and external organisations.
Applicants will have:
Experience of working with health needs and health inequalities. You should possess a supportive, patient, but determined nature and have a passion for supporting vulnerable people to enhance their lives within independent living. You should have the ability to build up a network of health professionals. You should also have a good track record of providing and delivering effective solutions within a housing environment or with vulnerable people. This post is exempt from the rehabilitation of Offenders Act 1974 and therefore will be subject to a DBS check from the Disclosure and Barring Service.
