This position conducts street-based outreach, develops individualized service plans, connects clients to housing and community resources, and provides ongoing case management to promote housing stability and improved health outcomes. The ideal candidate is comfortable conducting year-round outreach in encampments and other community settings and excels at building relationships with clients, healthcare providers, housing partners, and social service agencies. This role requires frequent community travel, strong communication skills, and a commitment to serving vulnerable populations with dignity and respect. Key ResponsibilitiesOutreach and Engagement Conduct year-round outreach to individuals experiencing unsheltered homelessness. Build trusting relationships using trauma-informed, person-centered approaches. Maintain a consistent presence in encampments, shelters, and other community settings. Provide transportation, as appropriate, to support housing, healthcare, and essential service access. Assessment and Care Planning Assess housing, healthcare, behavioral health, and social service needs. Identify barriers to stability and develop individualized care plans. Monitor progress and adjust plans as needed. Housing Navigation Assist clients in securing appropriate housing opportunities. Support housing applications, documentation, benefits enrollment, and move-in processes. Coordinate housing placements and successful transitions. Case Management and Care Coordination Provide ongoing case management, follow-up, and home visits as appropriate. Connect clients to healthcare, behavioral health, substance use treatment, and community resources. Coordinate services and advocate for client needs across systems. Documentation and Reporting Maintain accurate, timely, and confidential client records. Complete required documentation, data entry, and outcome tracking. Ensure compliance with organizational and regulatory requirements. Collaboration Build and maintain partnerships with healthcare, housing, and social service providers. Participate in interdisciplinary meetings and community collaboration efforts. Represent the organization professionally in all settings. QualificationsRequired High school diploma or equivalent; associate or bachelor's degree in social work, human services, public health, psychology, nursing, or a related field preferred. Ability to work independently and effectively manage a caseload. Strong communication, organization, and problem-solving skills. Experience with data entry and electronic record systems. Ability to conduct community outreach in all weather conditions. Valid driver's license, clean driving record, and reliable transportation. Preferred Experience working with adults experiencing homelessness. Knowledge of housing resources, supportive housing programs, and homeless services systems. Familiarity with mental health services, physical healthcare systems, assisted living resources, and community-based supports. Experience providing case management, care coordination, or outreach services. Understanding of trauma-informed care, motivational interviewing, and harm reduction practices.
