Job Description

The Reentry Support Specialist provides person-centered support to individuals transitioning from incarceration, helping them address behavioral health, housing, employment, and other reentry-related needs. This full-time position is responsible for conducting assessments, developing Individualized Service Plans (ISPs), delivering direct services, and connecting participants to critical community resources. Working within the

Reentry Program, the Specialist collaborates with internal staff and external partners to promote stability, reduce recidivism, and ensure compliance with program goals and reporting requirements. The ideal candidate is compassionate, detail-oriented, and committed to empowering justice-involved individuals through holistic, solution-focused support. Education and/or
Experience:
Associate or Bachelor's degree in social services, psychology, counseling, criminal justice, workforce development, or a related field preferred. Minimum of two years' experience in behavioral health, social services, workforce development, housing, corrections/reentry, or related field. Experience with assessments, service planning, and coordinating care for individuals facing multiple barriers (e.g., justice-involvement, homelessness, unemployment)
Certificates, Licenses, Registrations:
Possess a privately owned vehicle, a valid driver's license, and insurance. Must be able to access locations, at a moment's notice, that may not be accessible by public transportation.

Background check and driving record check.
