To Apply for this Job Click Here Requirements Bachelor's degree in Human Services or related field 1+ years of experience in case management, housing services, or placements Experience working with homeless or at-risk populations, including exposure to mental health or substance use Strong communication, documentation, and database/computer skills Job Duties Conduct intake, screening, and assessments to identify housing barriers, needs, and preferences Develop individualized housing/service plans in collaboration with Veterans and VASH case managers Provide short-term (up to 6 months) case management to support transition into VASH services Assist Veterans with housing search, placement, and use of Rapid Re-Housing to bridge delays Coordinate rental assistance, move-in logistics (furniture, transportation), and community resource connections Partner with VA Medical Centers, VASH teams, and community providers; attend case conferences and weekly calls Maintain accurate documentation, meet reporting deadlines, and ensure program compliance Additional Information Community-based role with regular travel throughout the greater Boston area Works closely with Veterans facing complex housing, financial, and behavioral health challenges Preferred experience with Housing First, Harm Reduction, and Trauma-Informed Care approaches Opportunity to build landlord relationships and support high-barrier housing placements
