Job Description

Position Title:
Community support specialists "peer recovery specialists".
Position Outline:
As Community support specialists (CSS); you will be able to work 40hrs per week, with assigned client's caseload of 20 to 25. CSS will be able to help people with disabilities, mental illness, and substance abuse challenges, CSS will be connecting clients with housing, healthcare, employment, and other vital services as outline in client IP (Individual Plan). An ideal Candidate will be helping individual who is struggling to navigate the social services system on their own due to their Mental health related issues, based on clients' goals; community support specialist serve as liaisons between clients and the community programs designed to help clients, locating services, and additional benefit; cutting through paperwork, and ensuring the right support is provided. CSS will be working as hybrid (office and community), scheduling clients for a visit in the community and returning to the office as soon as the daily visit is completed. CSS will pick one day out of the week to complete community outreach for new collaboration and potential clients lead.
Qualifications:
Community support specialists (CSS) must have a bachelor's degree in a health-related field or psychology or two to 3 years of working experience as peer recovery specialists. Community support specialists (CSS) should have knowledge of the Mental health Program and be able to demonstrate proficiency in creating and implementing Individual Plans (IP). Community support specialists (CSS) must be able to express themselves well through writing, interpersonal and oral communication skills. A Valid Maryland driver's license required; Experience working with Behavioral Health/Outpatients services preferred.
Supervision:
Community support specialists (CSS) report directly to the Program Director, the
Operational Manager or Program Rehabilitation Specialist, Essential Responsibilities:
1. Community support specialists (CSS). Implement on-site and off-site rehabilitation activities outlined in the Individual Plans (IP) for assigned clients of 20 to 25 caseload. 2. Community support specialists (CSS) implement the Individual Plans (IRP) based on the specific strengths, skills and needs of children or Adult on assigned caseload of 20 to 25 clients. 3. Community support specialists (CSS) Complete assigned in-home or community-based visits for each child and Adult on assigned caseload as scheduled each month, providing individualized, creative assistance and support designed to help children and Adult to achieve their identified goals. 4. Community support specialists (CSS) will be able to work with a wide range of clients with community Integrated. 5. Community support specialists (CSS) will be able to manage clients' physical or mental disabilities, individuals recovering from substance abuse, veterans, people exiting incarceration, and those experiencing homelessness. 6. Community support specialists (CSS) assess their clients' needs, then research and arrange the appropriate services. 7. Community support specialists (CSS) will handle paperwork, coordinate with agencies, follow up to ensure services are delivered, and advocate for clients when things fall through the cracks. 8. Community support specialists (CSS) ensure Client achieve the needs and goals by navigating resources for client in community. 9. Community support specialists (CSS) will be able to help clients with the following services: Safe and affordable housing, Vocational training and employment assistance, Transportation, Hospital discharge support, public assistance programs like food stamps, Medicaid, and housing vouchers, Community activities and peer support groups 10. Community support specialists (CSS) are also often on the front lines during client mental health crises. 11. When a client's situation becomes acute, Community support specialists may contact family members, reach out to law enforcement, or coordinate with a psychiatrist for hospital placement.
