Job Description

In this role, you will serve as the primary point of entry into the service system for individuals with developmental disabilities. You will guide clients through the initial application process, facilitate their long-term care planning, and monitor ongoing support services.
Key Responsibilities Application Assistance:
Help individuals prepare applications and gather the necessary documentation for the Developmental Disabilities Administration (DDA) to determine eligibility.
Care Planning:
Lead and facilitate the development of each client's Person-Centered Plan (PCP).
Monitoring & Oversight:
Coordinate, track, and monitor the PCP and its associated services to ensure quality care.
Individual Assessments:
Complete additional specialized assessments as required by the client's specific situation.
Compensation & Benefits Type:
Full-time contractual
Pay:
$26.71 - $34.56/hour This position is eligible for State of Maryland benefits.

An associate's degree with two (2) years of experience in a human services field.

High school diploma or equivalency certificate and seven (7) years of experience in a human services field.
