Job Description
Job Overview The Peer Support Specialist promotes independence, community integration, and stabilization for individuals experiencing mental health challenges. This role fosters an environment of acceptance, empathy, hope, and recovery, while supporting the principles and practice of dual recovery. Respect for consumer choice is central to service delivery, with services provided in the Saginaw community at times and locations chosen by the consumer. The Peer Support Specialist coordinates person-centered services that assist individuals in accessing needed medical, social, educational, and community-based supports. Serving as a liaison between consumers and the broader community, the Peer Support Specialist focuses on strengthening natural support systems and supporting consumers in developing and sustaining meaningful relationships that promote wellness and recovery Duties 1. Provide individual health and wellness coaching to support consumers in achieving personal health objectives and recovery goals. 2. Share lived-experience knowledge to coach and support others in managing mental and physical health. 3. Support consumers in developing skills to make informed, self-directed choices. 4. Collaborate with consumers to create, adapt, and refine wellness goals that address barriers to mental and physical health. 5. Coordinate with other Peer Support Specialists and Project ASSERT Advocates to expand the reach of wellness- and health-focused peer services. 6. Work closely with Saginaw County Community Mental Health Authority and community partners to engage consumers in appropriate services and resources and support coordinated care. 7. Promote consumer engagement in primary care, therapeutic services, psychiatric appointments, PBHCI registry compliance, and wellness activities. 8. Complete all required tracking, data collection, and reporting for evaluation and outcome measurement purposes. 9. Accurately document all job-related activities in accordance with agency policy using electronic medical records, as required. 10. Participate in staff meetings to receive guidance, share updates, and represent the voice of consumers and recovery-oriented perspectives. 11. Participate in approved staff development activities, trainings, in-services, and supervisory sessions. 12. Maintain appropriate professional boundaries and provide timely follow-up with consumers. 13. Deliver self-help recovery services and peer-led wellness supports. 14. SCCMHA. 15.
Maintain accurate, complete, and timely written or electronic records as required. 16. Implement case management services in support of treatment plans by aligning interventions with consumers' levels of motivation and readiness. 17.Link/Coordinate:
Arrange and support access to internal and external services, including mental health, physical health, safety, and ancillary supports, as needed or requested. 18.Educate:
Provide consumer-appropriate education that promotes informed choice, empowerment, and self-advocacy. 19.Advocate:
Advocate for consumer rights within the agency, partner agencies, and the community, while promoting consumer self-advocacy. 20.Monitor:
Assess the effectiveness of services and collaborate with consumers to adjust plans based on evolving needs. 21. Document and chart services appropriately for individuals with co-occurring disorders. 22. Ensure assurance, protection, and confidentiality of consumer rights and information at all times. Requirements Lived experience with behavioral health or addiction recovery, demonstrating resilience and insight Knowledge of crisis intervention, crisis management, and behavioral health principles Strong skills in mental health counseling, mentoring, and motivational interviewing techniques Experience working with social work practices or addiction counseling preferred Ability to operate EHR systems efficiently for documentation purposes Excellent communication skills with a positive, energetic attitude toward client engagement Certification or training in crisis intervention or related fields is a plus but not required a. Comply with all required initial and ongoing training requirements. b. Maintain up-to-date personnel documentation. c. Comply with all applicable federal, state, and local laws related to agency operations. d.
