Job Description

Job Summary Luther Haven seeks an organized, energetic and compassionate individual to be their Social Services Coordinator. This role is an integral part of the organization who works closely with residents, families, and staff. Duties Provide Social Services oversight completing duties as assigned for the residents of Luther Haven Intake for referrals at Luther Haven. Work closely with the Director of Nursing and Medical Records to coordinate referral-to-admission process. Assist new residents and family members with completion of admission paperwork. Provide information to residents/families about Medicare, Medicaid and other financial assistance programs available to the resident; assist with completion of Medicaid applications as deemed necessary. Ensure PASRR is completed to state requirements. Complete resident assessments and social history per MDS process and RAI manual. Schedule care conferences with residents and family members. Involve the resident and family members in planning objectives and goals for the resident. Develop individualized care plans to work towards meeting identified goals. Complete timely progress notes on each resident. Is the Grievance Officer for the facility. Works on investigations and helps identify grievance resolution. Assists the Administrator and Director of Nursing with OHFC reporting and investigating. Serve as an advocate for the residents and their rights. Completes private room, medically necessary forms. Updates elopement risk list. Participate in monthly QAPI Committee meetings. Attend daily IDT meetings, care conferences, and weekly Medicare meetings. Initiate and lead the discharge planning process; coordinate with outside resources such as: DME, Home Health, Hospice, etc. to ensure a safe transition to home or lower care setting. Assist in providing in-service education and orientation for employees regarding Resident Right and social service needs of residents. Provide notification to the Ombudsman regarding discharges. All other duties assigned by Administrator. Qualifications Experience in a long-term care setting. Knowledge of Medicare and Medicaid is preferred, but not required. Experience in social services preferred but not required. Able to promote cooperation between social services and other departments. Must have excellent communication skills with residents, families, and co-workers. Must be able to prioritize multiple tasks and meet work deadlines. Ability to write and give clear, concise instructions and remain calm during emergency situations.
Pay:
$24.00 to $32.00
Hours:
8:00am to 4:30pm
