Position Details Service Coordinator for subsidized housing working with elderly persons and persons with disabilities to foster an environment in which elderly persons and persons with disabilities can live independently and remain in their community. Is a resource for residents regarding available community-based services to assist residents in accessing community-based services and make referrals . Conduct initial and annual resident assessments, answer questions, and provide follow up regarding services. Facilitates wellness and other educational programs for residents. Proactive in engagement and outreach for new and existing residents by establishing partnerships with community-based service providers. Accurately document interactions with resident, family and service providers. Complete and submit required HUD reports. What You Offer Baccalaureate Degree-Concentration in Social Work, Gerontology, Psychology or Public Health preferred. Experience may substitute for the degree in part or whole. Experience in service management, including organizing, problem solving, and advocating. Training or experience in the aging process, elder services, disability services, drug and alcohol abuse, and mental & behavioral health diagnosis. Awareness of eligibility for and procedures of Federal and state entitlement programs. Good communication, writing, problem solving and organizational skills in addition to strong advocacy capabilities. Ability to effectively communicate in English, both verbally and in writing, and act in a positive, professional manner. Must be able to pass background study through the Minnesota Department of Human Services. Work Schedule Part-Time; 36 hours per biweekly pay period. Weekdays, 3 days a week from 8:30am-3:00pm.
