Qualified Professional (QP) Merciful Hands Day Program Greensboro, NC Job Details Full-time From $22 an hour 1 day ago Qualifications Sociology Bachelor's degree Psychology Human Services Social Work Full Job Description We are seeking a Qualified Professional to coordinate, implement, and monitor Rehabilitation (PSR) and Peer Support Services within the agency. The QP ensures that services are recovery-oriented, person-centered, and compliant with Medicaid, state regulations, and agency policies. This role involves overseeing PSR service and Peer Support Specialists, supporting service quality, reviewing documentation, coordinating care, and ensuring clients receive effective and ethical support services. Key Responsibilities PSR
  • Conduct ongoing assessments of client needs using clinical evaluations and input from clients, families, guardians, providers, and other supports to identify goals, strengths, preferences, and required services.
  • Participate in or facilitate person-centered treatment planning with clients, guardians, targeted case managers, and chosen support persons, while communicating progress and services with consent.
  • Develop, revise, and oversee individualized Person-Centered Plans (PCPs) with measurable goals, strategies, and treatment objectives appropriate to the client's diagnosis and assessed needs.
  • Ensure compliance with applicable NC Medicaid, DHHS, LME/MCO, CARF standards, and Merciful Hands Day Program policies and procedures.
  • Monitor client services and progress through site reviews, face-to-face contacts, and communication with families, staff, and providers to ensure quality care and appropriate level, frequency, and duration of services; document all monitoring activities.
  • Participate in and document crisis response interventions outlined in the PCP and manage client discharges according to DHHS and agency policies and procedures.
PEER SUPPORT
Provide supervision and guidance to Peer Support Specialists and direct care staff. Monitor service delivery to ensure compliance with agency standards and Medicaid requirements. Review and approve service documentation, progress notes, and treatment-related records. Ensure services are recovery-oriented, strength-based, and person-centered. Develop and revise individualized Person-Centered Plans (PCPs) and participate in the implementation of individualized service plans. Conduct orientation, training, coaching, and performance monitoring for Peer Support staff. Support staff in maintaining ethical boundaries and professional conduct. Monitor client progress and identify barriers to successful outcomes. Assist in crisis intervention and ensure appropriate follow-up and referrals. Education & Experience Bachelor's degree in Human Services, Psychology, Social Work, Counseling, Sociology, Human Services, or related field required. Minimum of 2 years' QP experience working in behavioral health, psychosocial rehabilitation, or community mental health services preferred. Experience supervising staff or coordinating behavioral health services strongly preferred. Knowledge & Skills Strong understanding of recovery-oriented and trauma-informed care. Knowledge of Medicaid documentation and behavioral health compliance standards. Excellent communication, leadership, interpersonal and conflict resolution skills. Ability to manage multiple responsibilities in a fast-paced environment. Ability to maintain professional boundaries and confidentiality Strong organizational and problem-solving abilities. Proficient in basic computer systems and electronic health record (EHR) documentation experience. Work Environment & Physical Demands Ability to maintain professional interactions in office, community settings and group facilitation. Ability to travel locally for client visits, meetings, and community-based services
  • Ability to stand, walk or sit for extended periods.
