Social Services Specialist Hillcrest Millard | Omaha, NE Mission-driven work. Meaningful impact.

Hillcrest Millard is seeking a compassionate and organized Social Services Specialist to support patients and families through every step of the care transition journey. This role is ideal for someone who thrives at the intersection of advocacy, coordination, and human connection. What You ll Do As a Social Services Specialist, you ll play a key role in ensuring safe, thoughtful, and well-coordinated transitions of care by: Meeting with patients and families at admission to complete paperwork and identify psychosocial needs Coordinating care transitions from admission through discharge in collaboration with the interdisciplinary team Completing referrals, assessments, care plans, progress notes, and required discharge documentation Communicating care recommendations and transition plans clearly with patients and families Providing counseling, education, and advocacy to protect patient rights and support decision-making Assisting with insurance-related matters including Medicare/Medicaid notifications, denials, and copays Responding to patient and family concerns promptly and professionally Educating care teams on identifying social service needs and available resources Participating in committees and meetings as assigned What We re Looking For Bachelor s degree in a healthcare-related field preferred Minimum of 2 years experience in social work, case management, or a related field Experience with discharge planning or care transitions in post-acute or acute care settings preferred Strong communication, organization, and documentation skills Ability to remain calm, flexible, and compassionate in emotionally charged situations Comfort using technology in a fast-paced healthcare environment Physical & Work Requirements Ability to lift up to 35 pounds Ability to sit, stand, bend, and move throughout the day Must be able to read, write, and speak English Possible exposure to communicable diseases and bodily fluids Why Hillcrest?
