Job Description

Are you passionate about helping Veterans achieve housing stability and long-term success? Do you thrive on building relationships, solving problems, and connecting people with resources that change lives? Harbor Care is seeking a compassionate, resourceful, and mission-driven Housing Specialist, SSVF to join our Supportive Services for Veteran Families (SSVF) team. In this role, you will work directly with Veterans and their families who are experiencing homelessness or are at risk of losing housing, helping them secure safe, stable, and affordable homes throughout Southern New Hampshire. As a Housing Specialist, you will serve as a critical link between Veterans, landlords, property managers, and community partners. You will help identify housing opportunities, navigate housing resources, and provide ongoing support to ensure successful housing placement and retention. Every day offers an opportunity to make a meaningful impact by helping Veterans overcome barriers and build a stronger future. What You'll Do Partner with Veterans and their families to identify and secure permanent housing opportunities. Build and maintain relationships with landlords, property managers, and housing providers throughout the community. Conduct housing assessments and match participants with appropriate housing options. Assist Veterans with housing searches, viewings, inspections, and lease-up activities. Support housing stability through ongoing follow-up, problem-solving, and connection to supportive services. Collaborate with case managers to develop individualized housing and service plans. Help resolve landlord-tenant concerns and advocate for successful housing outcomes. Maintain accurate documentation and housing data within HMIS and other agency systems. Ensure housing placements meet applicable federal, state, and local housing standards. Support program goals and compliance requirements associated with SSVF funding.
