The Senior Social Services Coordinator will: Determine whether individuals match the New Mexico definition of intellectual/developmental disability (IDD); Assist individuals and families through the allocation process, provide appropriate referrals to available resources; Conduct outreach for the program, and provide quality customer service; Assist individuals and families with their applications for Home and Community Based Waiver (HCBS) waivers provided through the Developmental Disabilities Supports Division (DDSD); Provide referrals to available services for individuals, families and other stakeholders and conducts outreach for the program. How does it get done? The Senior Social Services Coordinator is responsible for: Making determinations to ensure that the Developmental Disabilities Supports Division (DDSD) continues to process Developmental Disability Waiver applications and allocate individuals for services in a fair and timely manner; Completing applications on a weekly basis; Accurately maintaining of the Central Registry database; Answering applicant, guardian, and authorized representative questions specific to the applicant and general preservice information as well as provide referrals to appropriate entities for their specific region; Providing continuous quality improvement. The Pre-Service Specialist is mission critical to hold true to our core values, especially customer service and accountability, while serving individuals with developmental disabilities Ideal Candidate The Pre-Service Specialist is mission critical to hold true to our core values, especially customer service and accountability, while serving individuals with developmental disabilities Minimum Qualification Associate's Degree in Social Work, Psychology, Guidance and Counseling, Education, Sociology, Criminal Justice, Criminology, Family Studies/Services, Social Sciences, or Human Services and one (1) year of experience as a social work assistant and/or in social or community coordination, working with communities, working on health or social service related matters, social work/case management experience, and/or behavioral health care. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling three (3) years may substitute for the required education and experience. Employment Requirements Must possess and maintain a valid New Mexico Driver's License. Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico or must pass and receive a Defensive Driving Course Certificate as a condition of continued employment within 45 days of employment. Travel may be required with some occasional overnight stays. Successful completion of the Health Insurance Portability and Accountability Act (HIPAA). Working Conditions Work is performed in an office setting: late hours and weekends work may be required. Will be exposed to regular periods of video display terminal and keyboard usage and stressful situations. Travel may be required. Incumbent will work under stress and frequent time constraints.
