We are in search of a SOS Peer Specialist in Westchester County to join our team! Primary Duties:
The SOS Peer Specialist will conduct community outreach to unsheltered homeless persons. You will apply evidence-based interventions consistent with a Critical Time Intervention (CTI) approach and facilitate participants access to housing and other essential health and social welfare services. You will perform persistent and assertive outreach and engagement using strengths-based approaches. Continually assess the mental and social needs of participants and promote participants access to housing and other services conducive to their recovery and other desirable outcomes. Provide post-housing care coordination services. Requires fieldwork, on call coverage and a willingness to work flexible hours. Qualifications Minimum High School Diploma or equivalent or higher; New York State Certified Peer Specialist preferred. Required completion of an internship or comparable training in peer advocacy. Experience in providing advocacy or care coordination services for homeless persons or those with mental illness or substance use disorder preferred. Valid, insurable driver's license preferred, not required.
