Housing Navigator Ohio Adult Care Facilities Association Columbus, OH Job Details Full-time $54,000 - $56,000 a year 1 day ago Benefits Mileage reimbursement Health savings account Health insurance Dental insurance Paid time off Vision insurance Life insurance Retirement plan Qualifications Teamwork Master's degree Bachelor's degree Public Health Case management Full Job Description The Ohio Adult Care Facilities Association is seeking a passionate, enthusiastic, and knowledgeable Housing Navigator (Case Manager) for our statewide Housing Navigator program for individuals with SPMI. The primary role of the Housing Navigator is to assist individuals, family members, service providers, discharge planners, Home Operators etc... seeking housing for someone living with a mental illness in a supported environment. Client Intake and Assessment Assess client and/or caller needs and level of functioning to determine appropriateness and readiness for various services and housing based on established rules and protocols. Conduct assessments covering housing, mental health status, substance use, medical, financial, and supportive service needs. Individualized Service Planning Develop and implement personalized service plans with clients to support their housing goals. Provide information, linkages, and appropriate referrals to programs to best meet the individual's needs while navigating housing. Coordinate with community partners to connect clients to services addressing medical, behavioral health, and social needs. Housing Search & Placement Assist clients with locating Residential Facilities Class 2 or 3 based on their needs, managing applications, liaising with Home Operators, facilitating and supporting the client through the move-in processes including Resident Agreement education, resident rights, and responsibilities. Provide the individual with information on other types of housing and how to access it if not appropriate for a Residential Facility Class 2 or 3. Provide consistent support through quarterly check-ins, monitoring housing stability, and addressing barriers. Advocacy & Outreach Advocate on behalf of clients with government entities and community organizations to promote housing stability and access to services. Complete ongoing community outreach and education on Residential Facilities Class 2 and 3 to partners throughout the state Documentation & Reporting Maintain accurate and detailed client records, progress notes, and reports in compliance with program requirements Personal Character and Values Adherence to and enthusiasm for the mission, vision and core values and beliefs of the Ohio Adult Care Facilities Association High personal and professional integrity Appropriately handle sensitive information and maintain confidentiality Education and Professional Experience Bachelor's degree in social work, Human Services, Public Health, or related field and 5 years of case management experience or master's degree and 2 years of case management experience. Strong knowledge of statewide housing programs, community resources, social service systems, Medicaid programs, state and federal benefits, levels of care and care coordination requirements Proven experience working with individuals with complex needs such as mental illness and/or substance use disorders that may also be experiencing homelessness, exiting systems, hospitalizations, and/or experiencing a crisis Excellent case management skills including interviewing, assessment, individualized planning, crisis intervention, and deescalation Outstanding communication and interpersonal skills Ability to work both independently and as a collaborative team member Proficient in data entry and maintaining accurate records Experience working with diverse populations required Other Requirements Pass an FBI and BCI background check
